I got some really helpful feedback on how I might improve how I apply the FX etc. I had a few hours this morning to have another go and thought I'd share this here.

I agree with Malesor! The main problem here is that the girl reacts as if she has received a third hit, just before falling on the sofa. Digital blood squibs, anyway, are better than the first attempt!
