Yes, you would absolutely have to call Portland for a limo. There aren't really any that serve the Tillamook, Oregon area (I live in Portland myself) and they repeatedly mispronounced 'Oregon'...

The answer for why the credit sometimes will say something like "HARVEY ALAN MILLER as Dr. Norman Korman" is that for some reason or another, they had it in his contract to be worded that way. Particularly the case we see the "as" included. In that case, it comes down to readability and just making it sound less awkward. I mean, as long as you have the "as," you are forced to give both the actor and character in that order. You can't say "Dr. Norman Korman as HARVEY ALAN MILLER" since that would be wrong. "Dr. Norman Korman was played by HARVEY ALAN MILLER" just looks ridiculous. So, they had to make it as readable as they can. And they did keep the same naming convention of actor in caps, roll in lowercase, even with the inclusion of "as" making them have to reverse their order. But, there isn't actually any required formatting for a movie's credits. It comes down to the director/studio heads/producers/hot dog vendors want to do it a certain way. Anyway, it all just comes down to contracts and whatnot. Best not think too much about it, really.
