We saw that third monolith twice briefly in Season 5 episode 11, when the team first returned to the Lighthouse from the future. Fitz and May took the virtual guided tour of the Lighthouse and found all three monoliths together in the storage room where they met Noah.... and at the end of the episode the beacon General Hale used to set them up exploded in that same room.
