Los really freestyles on that Harry Mack level. Find a video where someone is throwing Los words its crazy. Juice wrld was crazy with freestyles, also I think Em is on that level too, not because its off the top but because he has 10,000+ writtens that nobody heard that he can pull from.

Freestyle doesn't mean off the top of the head. Freestyle is just a freeform rap session without any rules. It can be written, improvised, in and out of both, etc.
Granted, off the top freestyle is far more impressive than written freestyling.

This is still a freestyle, but they are definitely written bars with an intended meaning. It's not Mack's level of just making it up off the top of his head as he goes, but it doesn't have to be really.
Most freestylers, Los included, do write. They write bars all the time and then the freestyle comes when they put it together during recording. So their bars are written, but it's not a written song, they just go off of written bars committed to memory. The only purely off the top freestylers I know of that are still out there and relatively big are Harry Mack and Blind Fury. Those two completely compose new lyrics on the spot each time. They do have reused bars that they have memorized, but they are more used for filler to think of original content.
Then there are actual written rappers that have written freestyles and only really go outside of written on a cypher occasionally (Pretty much every mainstream rapper that has a "freestyle" falls into this category). It really just depends on what speaks to you more, but I am with you Stevie.. Harry Mack ruined freestyles for me too, I find it hard to even listen to big rapper's freestyles like Em and even Los too and he's really good. He's just not on the same level with the improv aspect of it and that's what speaks most to me.
