Ayanokoji may not be a hero and he definitely employs brutal methods but the dude is certainly efficient haha Also, he knew to lift her shirt and check there because he noticed that she instinctively protected her left side when he got close and started threatening her. Right over where her scar is. They do not show or describe it very well in the anime, but it is a very deep and gruesome scar that was likely a life threatening wound from a knife. Kei is a very proud girl who values beauty and hates the 'ugly' scar that she has on her body. She is insecure about it partly because she is worried about how men would react to it and view her after seeing it. Ironically, because Ayanokoji does not show any revulsion about her scar in that scene and even somewhat accepts it, she starts to view him differently and warm up to him. So although it was not really Ayanokoji's goal, that scene resulted in a big step forward in Karuizawa's mental state and her view of herself!
