I believe the 'Satoru' appearance he takes in this OVA is supposed to look like a younger version of what he looked like in his previous life, Mikami Satoru.

Because of this, the base of the mimic's appearance looks very similar to Shizu, however he can make the mimic look however he wants. It just takes extra effort that he ordinarily does not put in. Remember how he made his human form look older, and more masculine and feminine in the main series.
